2. ACCESS AND COMPLETE risk assessment form

a) Identify activity / task to be assessed and describe the potential hazard.

b) Who is likely to be at risk?

c) Assess likelihood and severity of risk from each identified hazard by allocating each a value from 1 to 3.

With '1' being the lowest and '3' the highest.

Further guidance on completing this section can be found in the key at the bottom of the form.

d) Allocate each hazard a 'risk rating' by multiplying the 2 values allocated in the previous step. Is the risk 'minor', 'medium' or 'high'?

If the risk is medium or high, control measures may be required.

e) What precautions or 'control measures' can be taken to reduce the risk? If necessary make a note of what further action is required - by whom and by when.

f) Review Date - make a note of a suitable review date for the risk assessment.
